RESUMO

BACKGROUND: Neonatal pulse oximetry screening (POS) algorithms for critical congenital heart disease (CCHD) have contributed towards decreasing neonatal mortality but cannot be applied at high altitudes. New POS algorithms at high altitudes are needed. METHODS: This observational, prospective study included newborns born at different altitudes from 0 to 4380 meters above the sea level in Peru. Healthy newborns underwent neonatal preductal and postductal oximetry, echocardiography and telephonic follow-up up to 12 months of age. Newborns with CCHD underwent preductal and postductal oximetry at the time of telemedicine evaluation while located at the high-altitude hospital where they were born, and their diagnoses were confirmed with echocardiography locally or after arriving to the referral center. Two new algorithms were designed using clinically accepted neonatal oximetry cutoffs or the 5th and 10th percentiles for preductal and postductal oximetry values. RESULTS: A total of 502 healthy newborns and 15 newborns with CCHD were enrolled. Echocardiography and telephonic follow-up were completed in 227 (45%) and 330 healthy newborns (65%), respectively. The algorithm based on clinically accepted cutoffs had a sensitivity of 92%, specificity of 73% and false positive rate of 27% The algorithm based on the 5th and 10th percentiles had a sensitivity of 80%, specificity of 88% and false positive rate of 12%. CONCLUSIONS: Two algorithms that detect CCHD at different altitudes had adequate performance but high false positive rates.

Introduction: The coronavirus disease 2019 (COVID-19) pandemic has triggered a rise in the global phenomenon of self-medication. The use of medicines with unsubstantiated claims of benefit for the prevention and/or treatment of infected people has been widely adopted. Objective: To determine the factors associated with the use of medicines and self-medication by Latin American populations during the COVID-19 pandemic. Methods: This analytical cross-sectional study was conducted via an analysis of secondary data collected from 8777 responses from 12 Latin American countries obtained from a survey on the use of medicines and self-medication during the pandemic. Results: Respondents who reported using Paracetamol, Ibuprofen, or antibiotics (ABx) disclosed this as self-medication in 26.9%, 16.6%, and 9.7% of cases respectively. In our multivariate analysis, significant associations were found between: self-medication by individual choice and the use of Hydroxychloroquine, ABx, or non-steroidal anti-inflammatory drugs (NSAIDs) (p = 0.026, = 0.003, and <0.001 respectively); self-medication on the recommendation of a family member, with ABx, Paracetamol, Ibuprofen, or other NSAIDs (p <0.001, <0.001, <0.001, and <0.001 respectively); and self-medication on the recommendation of another person, with Chloroquine, Hydroxychloroquine, Azithromycin, Penicillin, other ABx, Warfarin, Ivermectin, Paracetamol, Ibuprofen, or other NSAIDs (p <0.001, <0.001, <0.001, = 0.001, <0.001, <0.001, = 0.004, <0.001, <0.001, and <0.001 respectively). Conclusion: There was a significantly high prevalence of self-medication in our Latin American study population, including the use of medicines not recommended for COVID-19 treatment and/or prevention. The implementation of public health measures aimed at combating the worrying COVID-19 infodemic is essential to prevent this ongoing issue and its associated negative impacts on both current public health and future medication efficacy. This is especially important in the case of ABx due to the threat of a future antimicrobial resistance pandemic.

Introducción:En el marco de esta pandemia, el personal de salud ha sufrido cambios psicológicos, debido a su relación directa con pacientes contagiados por COVID-19, convirtiéndose así en parte de un grupo vulnerable.Objetivo:Conocer los factores asociados a los trastornos de la esfera mental en el personal de salud amazónico durante la pandemia COVID-19.Material y Métodos:La población estuvo conformada por el personal de salud de dos establecimientos de I y II nivel, donde la muestra fue de 341 personas. El tipo de estudio es observacional, transversal analítico, se usó como instrumento una encuesta auto aplicada.Resultados:Se encontró mayor depresión en mujeres (RPa: 1,71; IC95%: 1,29-2,26; valor p<0,001) y menor depresión con tener mayor edad (RPa: 0,98; IC95%: 0,97-0,99; valor p=0,018). La ansiedad se asoció con tener familiares fallecidos (RPa: 1,06; IC95%: 1,01-1,12; valor p=0,029). El tener enfermedades previas (RPa: 1,72; IC95%: 1,27-2,32; valor p<0,001), tener colegas fallecidos (RPa: 1,05; IC95%: 1,05-1,06; valor p<0,001) y ser enfermera (RPa: 1,40; IC95%: 1,05-1,85; valor p=0,020) tuvo estrecha relación con el estrés post traumático (EPT).Conclusión:Se concluye que existe factores asociados a la depresión, ansiedad, estrés y EPT en el personal de salud amazónico durante la pandemia COVID-19. Introduction:In the context of this pandemic, healthcare personnel have experienced psychological changes, due to their direct contact with COVID-19 infected patients, becoming part of a vulnerable group.Objective:To identify factors associated with mental health disorders among amazonic healthcare personnel during the COVID-19 pandemic.Material and Methods:The population consisted of healthcare personnel from two level I and II establishments, with a sample size of 341 individuals. The study design was observational, cross sectional, and a self-administered survey was used as the instrument.Results:A higher prevalence of depression was found in women (adjusted prevalence ratio [aPR]: 1.71; 95% confidence interval [CI]: 1.29-2.26; p-value <0.001), and a lower prevalence of depression was associated with older age (aPR: 0.98; 95% CI: 0.97-0.99; p-value=0.018). Anxiety was associated with having deceased family members (aPR: 1.06; 95% CI: 1.01-1.12; p-value=0.029). Having previous illnesses (aPR: 1.72; 95% CI: 1.27-2.32; p-value <0.001), having deceased colleagues (aPR: 1.05; 95% CI: 1.05-1.06; p-value <0.001), and being a nurse (aPR: 1.40; 95% CI: 1.05-1.85; p-value=0.020) was closely related to post traumatic stress (PTSD).Conclusion:It is concluded that there are factors associated with depression, anxiety, stress, and PTSD among amazonic healthcare personnel during the COVID-19 pandemic. INTRODUCTION AND OBJECTIVES: Burnout syndrome (BS) has been evaluated in few populations of medical students, and its relationship with depression is even less studied. The objective was to determine the frequency of BS in medical students of the Universidad Pedagógica y Tecnológica de Colombia (UPTC), in Tunja, Colombia, as well as its association with depression and other social, economic, demographic, and academic factors. METHODS: An observational, analytical, cross-sectional study was carried out on 182 UPTC medical students during 2018. The Maslach Burnout Inventory-Student Survey (MBI-SS) was applied, with which 3 components were obtained to determine positive SB. This was crossed in the Stata 15 program for depression and other social, economic, demographic, and academic covariates with the Generalised Linear Model (GLM). RESULTS: Of 182 respondents, 51.4% were women. The median age was 21 years (20-23 years). 14% had BS, of which 48% had depression. In the multiple regression, BS was significantly associated with a higher frequency of depression (RPa = 5.54; IC95%, 2.36-13.02; P < 0.001) and the feeling of insufficient money (RPa = 4.37; IC95%, 1.95-9.83; P < 0.001), in contrast to a negative association with smoking (RPa = 0.13; IC95%, 0.06-0.30; P < 0.001) and being a woman (RPa = 0.32; IC95%, 0.12-0.82; P = 0.018). The age of onset of marijuana use was not significant. CONCLUSIONS: BS shows a high association with increased depression and a feeling that the money is not enough at the end of the month, but it showed a negative association with tobacco consumption and being a woman. Such students should be detected to provide them with adequate academic support.

Introduction and objective: Sci-Hub is a website that allows users to download full-text versions of millions of scientific articles for free. The objective of this study was to evaluate the association between the use of Sci-Hub and consultation of scientific journals by medical students from six Latin American countries. Methods: We conducted a secondary analysis of data from a 2017 cross-sectional study of medical students from six Latin American countries (Argentina, Bolivia, Chile, Colombia, Paraguay, and Peru). Consultation of scientific journals was considered as the dependent variable, while the independent variable was the use of Sci-Hub. Responses were categorized as: "do not know"; "did not use it"; "used it at least once a week"; "used it more than once a week"; and "used it every day of the month". In simple and multiple regression analyses, multivariate random-effects models were used to estimate prevalence ratios (PR), with 95% confident intervals (CI). Results: Of the 6632 participants, 38.2% consulted scientific journals and 10.3% used Sci-Hub once a week. Using Sci-Hub at least once a week was associated with a 20% increase in the prevalence of consulting scientific journals (PR: 1.20, 95% CI: 1.10-1.31, p < 0.001). The variables positively associated with Sci-Hub use included being in the sixth year of medical school (PR: 2.34), affiliation to more than one academic research group (PR: 1.81), being a medical student in Colombia (PR: 1.63), intermediate (PR: 1.16) and advanced levels of English (PR: 1.23), and daily use of PubMed (PR: 1.66), SciELO (PR: 1.87), and/or SCOPUS (PR: 1.58). Conclusion: Amongst medical students surveyed from the above six Latin American countries, the use of Sci-Hub at least once a week was significantly associated with the self-reported prevalence of consulting scientific journals.

Sustainability may be at risk in a population that has altered health, according to Sustainable Development Goal 3 (SDG 3): Health and well-being. The ongoing conflict between Russia and Ukraine could jeopardize SDG 3, specifically the mental health of the population. The present study sought to determine the association between severe anxiety, depression and stress in population of 13 Latin American countries according to fear about the war conflict. It was a cross-sectional, analytical and multicenter study. Anxiety, depression and stress were measured with the DASS-21 test (Cronbach's Alpha: 0.97) and fear due to an armed crisis with a questionnaire already validated in Latin America (Cronbach's Alpha: 0.92), which was also adjusted for sex, age, education level and country of residence. Descriptive and analytical statistics were obtained. Of the 2,626 respondents, the main fear was that weapons of mass destruction would be used. In the multivariate models, strong associations were found between fear of a possible world-scale armed conflict and having severe or very severe levels of anxiety (aPR: 1.97; 95% CI: 1.64-2.36; value of p <0.001), depression (aPR: 1.91; 95% CI: 1.54-2.36; value of p <0.001) or stress (aPR: 2.05; 95% CI: 1.63-2.57; value of p <0.001). Sustainability linked to SDG 3, specifically mental health, is affected by this type of significant events, given the possible global war crisis that could trigger major events, even more so if added to the deterioration already experienced by COVID-19 in the Latin American region, insecurity and constant political uncertainty.

During the pandemic, there has been evidence of work limitations during telework, which are believed to cause mental health problems. Our objective was to assess the association between perceived work limitations during telework and posttraumatic stress during the COVID-19 pandemic in Latin America. A cross-sectional study was conducted in Latin America in 2020. Exposure was measured by self-reporting using a self-perception questionnaire; the SPRINT-E questionnaire was used for outcome measurements. Generalized linear models were applied. Of 1329 participants, 15.2% (n = 202) had posttraumatic stress. In a multivariate analysis, the highest frequency of posttraumatic stress was found among those with moderate depression or more (PR = 1.29; 95% CI: 1.03-1.61), moderate or more anxiety (PR = 2.34; 95% CI: 1.61-3.41), and moderate or more stress (PR = 2.45; 95% CI: 1.46-4.12). In conclusion, there is an association between perceived work limitations during telework and posttraumatic stress in Latin American workers. For this reason, it is recommended that occupational physicians, companies, and institutions assess the frequency of posttraumatic stress and monitor the mental health of workers.

Introduction: The COVID-19 pandemic contributed to the spread of abundant misinformation by the media, which caused fear and concern. Objective: To determine the association between the pathologies of the mental sphere and the perceptions of fear and exaggeration transmitted by the media with respect to COVID-19 in Latin America. Methodology: The present study has an analytical cross-sectional design that is based on a validated survey to measure fear and exaggeration transmitted by the media and other sources (Cronbach's α: 0.90). We surveyed more than 6,000 people, originally from 12 Latin American countries, who associated this perceived exaggeration with stress, depression, and anxiety (measured through DASS-21, Cronbach's α: 0.96). Results: Social networks (40%) or television (34%) were perceived as the sources that exaggerate the magnitude of the events. In addition, television (35%) and social networks (28%) were perceived as the sources that generate much fear. On the contrary, physicians and health personnel are the sources that exaggerated less (10%) or provoked less fear (14%). Through a multivariate model, we found a higher level of global perception that was associated with whether the participant was older (p = 0.002), had severe or more serious anxiety (p = 0.033), or had stress (p = 0,037). However, in comparison with Peru (the most affected country), there was a lower level of perception in Chile (p < 0.001), Paraguay (p = 0.001), Mexico (p < 0.001), Ecuador (p = 0.001), and Costa Rica (p = 0.042). All of them were adjusted for gender and for those having severe or major depression. Conclusion: There exists an association between some mental pathologies and the perception that the media does not provide moderate information.

Introduction: During the covid-19 pandemic, mental health services were unable to cope with the high demand from the population, so many people chose to search the Internet for information that could help them cope with the psychological process they were experiencing at the time. The aim of this study was to characterize the global search trend for the term «psychiatry¼ in the context of covid-19 using Google Trends and Our World in Data. Methods: Descriptive-cross-sectional study on global search trends for Psychiatry in the context of covid-19 under the terms «psychiatry¼, «depression¼, «anxiety¼, «stress¼, «insomnia¼ and «suicide¼ in the category of health, this was conducted over the period 2020-2021 and time graphs were generated. Results: The term «psychiatry¼ remained at a consistently high relative search volume (between 60 and 90), with a significant and gradual search in the month of April. The relative search volume for «depression¼, «anxiety¼ and «stress¼ remained constant with some non-significant fluctuations over the period 2020-2021. The term «insomnia¼ was predominant between January and June 2020, gradually declining in April and remaining constant until October 2021. Finally, the term «suicide¼ had a fluctuating RBV between 60 and 100 during this period. Conclusions: During the study period, the topics related to mental health and the speciality of psychiatry remained constant, with some fluctuating, but not outstanding variations.

Introducción: Es importante identificar variables de protección de la salud cognitiva-psicológica en pacientes dados de alta por COVID 19, específicamente que mitiguen las quejas cognitivas de memorias, sobre todo si es la actividad física,que ayuda a mantener el peso y da muchos beneficios más.Objetivo: Determinar la asociación entre el malestar psicológico y las quejas subjetivas de memoria, en paciente dados de alta por COVID 19, y el rol de la actividad física como variable moderadora. Metodología: Estudio correlacional multivariable, basado en un formulario online, en 299 personas que se contagiaron de la enfermedad COVID 19. Se utilizaron los siguientes instrumentos DASS 21, cuatro preguntas para evaluar quejassubjetivas de memoria e ítems específicos de una escala deEstilo de vida saludables. Los análisis de moderación se hicieron a través del comando PROCESS22 en SPSS V.23. Resultados: Se muestran asociaciones negativas significativas del ejercicio físico y el malestar psicológico en término de ansiedad (r= -0,21**) y depresión (r=-0,13**), además de evidenciar que el efecto conjunto de ambas variables e ssignificativo y negativo frente a las quejas cognitivas subjetivas de memoria (Ansiedad /Ejercicio físico = β = −0.08, p <0,05; Depresión /Ejercicio físico = β = −0,08, p < 0,05). Conclusiones: Se confirma el rol de moderación del ejercicio físico, la variable se comporta como factor de protección de la salud cognitiva-psicológica en pacientes dados de alta por COVID-19.(AU)


Introduction: Identifying protection factors that preservecognitive-psychology health in COVID 19 recovered-patientsis important. Specifically, those that mitigate subjective memory complaints, punctually physical Exercise helps to maintaina healthy weight and much more benefits. Objective: To determine the association between psycho-logical distress and subjective memory complaints in COVID19 recovered-complaints and physical exercise as a moderatevariable. Methodology: Correlational multivariable study, with anonline form. Results: In 299 recovered-patients, negative associationsbetween physical exercise and psychological distress arefound in terms of anxiety (r=-0.21**) and depression (r=-0.13**), additionally the moderate effect of both variables inthe subjective memory complaints is significant (Anxiety/Physical Exercise = β = −0.08, p < 0.05; Depression/Physical Exercise = β = −0.08, p < 0.05). Conclusion: independent moderation role of physical exercise and mindfulness was tested and determined. Both variables could be identified as protection factors.(AU)

Introducción: Para policías y militares, el acceso a información idónea y veraz siempre ha sido importante; durante la pandemia la necesidad fue aún mayor debido a las labores que les fueron asignadas, como parte de la primera línea frente a la pandemia por la COVID-19. Objetivo: Determinar los factores asociados al miedo por la información recibida durante la pandemia en policías y militares peruanos. Métodos: Estudio transversal con técnicas analíticas, se encuestó a 1 017 policías y militares peruanos durante la pandemia, para ello se usó un test validado para medir la percepción de miedo o preocupación que transmitían los medios de comunicación, se informaron las frecuencias y resultados analíticos. Resultados: Los encuestados percibieron mucho miedo a las redes sociales (22 % muy de acuerdo y 36 % de acuerdo) y la televisión (20 % muy de acuerdo y 32 % de acuerdo). En el modelo multivariado, los militares percibían menos miedo en comparación de los policías (RPa: 0,65; IC95 %: 0,49-0,88; valor p= 0,004), además, internet fue el medio que generó una mayor frecuencia de percepción de miedo entre los encuestados (RPa: 1,65; IC95 %: 1,04-2,61; valor p= 0,032), ajustado por 3 variables. Conclusión: La televisión y las redes sociales son los principales medios que generaron miedo entre los policías y militares; además, el estar asustado estuvo más asociado a informarse por internet.


Introduction: For police and military, access to suitable and truthful information has always been important, during the pandemic the need was even greater due to the tasks assigned to them as part of the front line against the COVID-19 pandemic. Objective: To determine the factors associated with fear of pandemic information received by Peruvian police and military personnel. Methods: Cross-sectional study with analytical procedures, 1 017 Peruvian police and military were surveyed, through surveys conducted during the pandemic, a validated test was used to measure the perception of fear or concern transmitted by the media, the frequencies and analytical results were reported. Results: The respondents perceived a lot of fear of social networks (22% strongly agree and 36% agree) and television (20% strongly agree and 32% agree). In the multivariate model, the military perceived less fear compared to the police (aPR: 0.65; CI95%: 0.49-0.88; value p= 0.004), in addition, the Internet was the medium that generated a greater frequency of perception of fear among the respondents (aPR: 1.65; CI95%: 1.04-2.61; value p= 0.032), adjusted for 3 variables. Conclusion: Television and social networks are the main media that generate fear among police and military; in addition, being scared was more associated with being informed through the Internet.

Introducción: La pandemia afectó el internado de estudiantes de salud en su asistencia hospitalaria, en la cual realizan las prácticas. Objetivo: Determinar los factores asociados a la percepción de repercusiones del internado hospitalario peruano durante la COVID-19. Métodos: Estudio transversal; se obtuvo estadística descriptiva, analítica bivariada y multivariada basado en una encuesta realizada de forma exploratoria, como parte de otra investigación mayor, a la cual se le asoció variables como edad, sexo, tipo de universidad, estrés, ansiedad, depresión y repercusiones del internado. Resultados: De los 121 encuestados, la mayoría pensó que su familia le hubiese apoyado si se enfermaba de gravedad, que pudo haber infectado a sus familiares o alguna amistad/conocido si acudían al hospital. Muchos estuvieron en desacuerdo con que el hospital o la universidad les darían los implementos de seguridad, por la falta de apoyo de estas entidades. Hubo asociación entre el pensar que se habría contagiado de la COVID-19 si iba al hospital versus el puntaje de ansiedad (p= 0,030), el sexo masculino, según la percepción de que la universidad les daría los materiales para cuidarse (p= 0,029) y el provenir de una universidad privada según el percibir que el hospital no le hubiese apoyado, si se enfermaba de gravedad (p= 0,049). Conclusión: Los factores asociados a la percepción de repercusiones del internado son el apoyo familiar, el poder contagiar a sus familiares, falta de materiales dados por las universidades y hospitales, la ansiedad, el sexo masculino y provenir de universidades privadas.


Introduction: The pandemic affected the internship of health sciences students when they attended the hospital. Objective: To determine the factors associated with the perceived impact of the Peruvian hospital internship during COVID-19. Methods: Cross-sectional; descriptive, bivariate analytical and multivariate statistics were obtained based on an exploratory survey conducted as part of a larger investigation, to which variables such as age, sex, type of university, stress, anxiety, depression, and repercussions of the internship were associated. Results: Of 121 respondents, most of them thought that family would have supported if they have gotten a severe disease. Also, they thought that they could have infected their families or friends/acquaintances if they have gone to the hospital. Many of them disagreed on the statement about the hospital or university providing them safety implements. There was an association between thinking that they could have gotten COVID-19 if they have gone to the hospital versus anxiety score (p= 0,030), being a male according to the perception that the university provided materials to take care of themselves (p= 0,029), and having studied in a private university according to perceiving that the hospital would not have supported them if they have gotten seriously ill (p= 0,049). Conclusion: The factors associated with the perception of repercussions of the boarding school are family support, being able to infect their acquaintances, lack of materials provided by universities and hospitals, anxiety, being male, and coming from private universities.

Introducción: Existe evidencia sobre el efecto negativo del uso de tecnologías de información y comunicación en estudiantes, sin embargo, poco se conoce sobre problemas fisioergonómicos secundarios a su uso. Objetivo: Identificar los factores tecnológicos-educativos asociados al reporte de 4 problemas fisioergonómicos en estudiantes de medicina. Métodos: Estudio transversal en estudiantes de medicina de 11 países de Latinoamérica y el Caribe. Se utilizó un cuestionario cuyas variables fueron las características socio-educativas y tecnológicas, así como los problemas fisioergonómicos (dolor de cuello, dolor de espalda, ojo seco/rojo y dolor de cabeza). Resultados: De 11 587 estudiantes, 14,5 % reportó presentar 4 problemas fisioergonómicos. La mayoría reportó dolor de cuello (50 %), dolor de espalda (50,5 %) y dolor de cabeza (53,7 %). Tuvieron mayor frecuencia de presentar estos problemas fisioergonómicos las mujeres (RP= 1,06), quienes estudiaron una carrera previa (RP= 1,19), pertenecer a todos los años de estudios (RP= 1,12-1,20), quienes tenían mayor uso de Internet en horas (RP= 1,01) y aquellos que accedían a Twitter en horas de clases (RP= 1,30). La frecuencia de presentar estos problemas disminuyó en quienes tenían menor edad en años (RP= 0,99), procedían de una universidad privada (RP= 0,81), y quienes pertenecían a cualquier grupo extracurricular (RP= 0,67-0,93). Conclusiones: Los factores tecnológicos-educativos asociados al reporte de 4 problemas fisioergonómicos en estudiantes de medicina son ser mujer, tener carrera previa, pertenecer a todos los años de estudios, horas de uso de internet y el acceso a Twitter durante clases.


Introduction: There is evidence on the negative effect of the use of information and communication technologies on students, however, little is known about physio-ergonomic problems secondary to their use. Objective: To identify the technological-educational factors associated with the report of 4 physio-ergonomic problems in medical students. Methods: Cross-sectional study in medical students from 11 countries in Latin America and the Caribbean. A questionnaire was used whose variables were socio-educational and technological characteristics, as well as physio-ergonomic problems (neck pain, back pain, dry/red eye and headache). Results: Of 11 587 students, 14.5% reported presenting 4 physio-ergonomic problems. Most reported neck pain (50%), back pain (50.5%), and headache (53.7%). Women (PR= 1.06), who studied a previous degree (PR= 1.19), who belonged to all years of studies (PR= 1.12-1.20), who had greater use of the Internet, had a higher frequency (in hours) of presenting these physio-ergonomic problems (PR= 1.01) and those who accessed Twitter during school hours (PR= 1.30). They decreased the frequency of presenting these problems, being younger in years (PR= 0.99), coming from a private university (PR= 0.81), and who belonged to any extracurricular group (PR= 0.67-0.93). Conclusions: The technological-educational factors associated with the report of 4 physio-ergonomic problems in medical students were being a woman, having a previous degree, belonging to all the years of study, hours of Internet use and access to Twitter during classes.

Objective: To determine the factors associated with complications of foreign body ingestion and/or aspiration in children from a hospital in the Peruvian social security program. Materials and methods: An observational, retrospective, analytical, and transverse study was undertaken. Medical records of patients under the age of 14 years old, who were admitted to the National Hospital Edgardo Rebagliati Martins between January 2013 and May 2017, and treated with a diagnosis of foreign body in the digestive or respiratory tract, were selected. Variables that characterized the foreign body ingestion and/or aspiration were assessed. STATA v11.1 was used for all subsequent statistical analyzes. Results: A total of 322 cases met the inclusion criteria and the median age of the cohort was 4 years old (interquartile range: 2-6). The most frequently ingested foreign bodies were coins (∼59%) and batteries (∼10%). Fifty-four cases (∼17%) were classed as having a complication. In the multivariate analysis, we observed that the frequency of complications increased when the ingested object was a battery (adjusted prevalence ratio (aPR): 2.89; 95% confidence interval (CI): 2.52-3.32; p-value<0.001), when the time elapsed prior to diagnosis was 8-16 h (aPR: 2.23; 95% CI: 2.18-2.28; p-value<0.001), and when the child was male (aPR: 1.85; 95% CI: 1.24-2.74; p-value = 0.002). However, the frequency decreased in cases where foreign bodies were lodged in the nose (aPR: 0.97; 95% CI: 0.97-0.98; p-value<0.001). Conclusions: Whilst the most frequently ingested foreign bodies in this study were coins, complications were more common in cases of battery ingestion and in those where the diagnosis was made after 8 h.

Marijuana is the most widely used illicit drug in the world, especially among young people. This study is relevant to policy makers because it expands the knowledge regarding drug use in vulnerable youth, allowing health authorities to reduce marijuana consumption via educational, family, and governmental strategies and policies. The objective of this study was to determine the prevalence of frequent marijuana consumption and its associated factors in young people before admission to juvenile detention centers in Peru. The data was taken from the 2016 National Population Census of the Youth Diagnostic and Rehabilitation Centers in Peru. The final sample was made up of 1,848 people with ages between 14 and 22 years old, with a median age of 17 (95.6% males). The variable frequent marijuana consumption was defined as the use of marijuana at least once a week, prior to entering the center. The main factors associated with frequent marijuana use were male sex, running away from home before the age of 15, physical abuse during childhood, having a family member who consumed alcohol or drugs frequently, and the presence of criminal gangs in the housing area. Additionally, it was found that living with parents up to a specific critical age decreases the probability of frequent use of marijuana in young people. These results could aid the development of strategies and public policies that help prevent the consumption of marijuana and other drugs from an early age.


La marihuana es la droga ilícita más consumida en el mundo, especialmente entre jóvenes. El presente estudio es relevante para la toma de decisiones en salud porque expande el conocimiento sobre el uso de drogas en la juventud vulnerable y permite a las autoridades sanitarias reducir el consumo de marihuana mediante estrategias educativas, familiares y gubernamentales. El objetivo de este estudio fue determinar la prevalencia del consumo frecuente de marihuana y sus factores asociados en jóvenes antes de su ingreso a centros juveniles de diagnóstico y rehabilitación en Perú. Los datos fueron tomados del Censo Nacional de Población en los Centros Juveniles de Diagnóstico y Rehabilitación del año 2016 en Perú. La muestra final estuvo compuesta por 1848 personas entre 14 y 22 años, con una mediana de edad de 17 años (95,6 % hombres). La variable consumo frecuente de marihuana fue definida como el consumo de marihuana de al menos una vez por semana por parte de los jóvenes, previo a su ingreso al centro. Los principales factores asociados al consumo frecuente de marihuana fueron el sexo masculino, huir de casa antes de los 15 años, haber sufrido abuso físico durante la infancia, tener un miembro de la familia que consuma alcohol o drogas frecuentemente y la presencia de pandillas criminales en la zona residencial. Asimismo, se halló que vivir con los padres hasta cierta edad crítica disminuye la probabilidad de consumo frecuente de marihuana en jóvenes. Estos resultados podrían ayudar a desarrollar estrategias y políticas públicas que ayuden a prevenir el consumo de marihuana y otras drogas desde edades tempranas.

Introduction: Latin America was the region most affected by COVID-19 in the second quarter of 2020, and consequently, the impact on mental health requires evaluation. The aim of this study was to assess the risk of post-traumatic stress disorder (PTSD) caused by bereavement due to COVID-19 in 12 countries in Latin America. Methods: The current study was an analytical cross-sectional study. Validated tests were applied for PTSD, depression, anxiety, and stress (DASS-21), questions about the respondent's condition or their environment, and demographic questions, as well as the length of the mourning period of suffering. Results: The outcomes demonstrated that the PTSD risk increased for women (p < 0.001), when a friend or acquaintance had COVID-19 (p = 0.002), when a close relative died from COVID-19 (p = 0.010), having severe depression (p <0.001), severe anxiety (p <0.001), severe stress (p <0.001), residing in Chile (p <0.001), Paraguay (p <0.001), Bolivia (p <0.001), Costa Rica (p <0.001) or El Salvador (p = 0.005). On the other hand, there was less risk of PTSD at an older age (p <0.001) or if respondents had a sentimental partner (p = 0.025). In the case of severe PTSD, there was a greater gender risk for women (p <0.001), a close relative dying from COVID-19 (p = 0.017), having severe depression (p <0.001), severe anxiety (p <0.001), severe stress (p <0.001), residing in Chile (p <0.001), Paraguay (p <0.001), Bolivia (p <0.001) and Costa Rica (p = 0.002). It was also observed that there was less risk of severe PTSD at an older age demographic (p <0.001). Discussion: It can be concluded that the percentages of PTSD are high in its clinical presentation as severe, especially among Latin American women.

Introducción: Algunas creencias religiosas conflictivas se han convertido en una amenaza para la salud pública en América Latina en tiempos de COVID-19. Objetivo: Revalidar una Escala de Fatalismo Religioso ante el COVID-19 en adultos de Latinoamérica. Métodos: Se realizó un estudio instrumental. Bajo un muestreo no probabilístico intencional participaron 6206 adultos, 60,1 por ciento mujeres de entre 18 a 60 años de edad (Media = 35,11; Desviación estándar = 9,59) de 14 países de Latinoamérica. El instrumento objeto de revalidación fue la Escala Modificada de Fatalismo Religioso ante la COVID-19 para adultos peruanos, la cual está compuesta por nueve ítems y cinco opciones de respuesta en formato Likert. La validez de contenido se analizó mediante el V de Aiken, la estructura interna a través del análisis factorial confirmatorio y la confiabilidad con el Alpha de Cronbach. Resultados: Todos los ítems obtuvieron una evaluación favorable (V > 0,70). En el análisis factorial confirmatorio, la falta de ajuste se mejoró con la técnica de modificación de índices, que orientó a eliminar los ítems 2, 7 y 8; de este modo se obtuvieron índices de bondad de ajuste satisfactorios, lo cual avala un modelo unidimensional conformado por 6 ítems. Por último, la confiabilidad fue aceptable (α = 0,89; IC 95 por ciento = 0,88-0,89). Conclusiones: La Escala de Fatalismo Religioso ante la COVID-19 es una medida breve que demuestra evidencias de validez y confiabilidad; por lo tanto, puede ser aplicada en investigaciones a nivel de Latinoamérica(AU)


Introduction: Some conflictive religious beliefs have become a threat to public health in Latin America in times of COVID-19. Objective: To revalidate a religious fatalism scale before COVID-19 in adults from Latin America. Methods: An instrumental study was conducted. Under a nonprobabilistic purposive sampling, 6206 adults participated, 60.1 percent of which were women aged 18-60 years (mean: 35.11; standard deviation: 9.59) from 14 Latin-American countries. The instrument to be revalidated was the modified religious fatalism scale before COVID-19 for Peruvian adults, which is made up of nine items and five response choices in Likert format. Content validity was analyzed using Aiken's V; internal structure, through confirmatory factor analysis; and reliability, with Cronbach's Alpha. Results: All the items obtained a favorable evaluation (V > 0.70). Concerning the confirmatory factor analysis, the lack of fit was improved with the index modification technique, which led to eliminating items 2, 7 and 8; thus, satisfactory goodness-of-fit indices were obtained, which supports a unidimensional model made up of 6 items. Finally, reliability was acceptable (α= 0.89; 95 percent CI: 0.88-0.89). Conclusions: The religious fatalism scale before COVID-19 is a brief measure that shows validity and reliability evidences; therefore, it can be applied in research at the Latin-American level(AU)

Introducción: Los profesionales de la salud ocupacional (SO) han tenido un rol importante en la vigilancia, prevención y control frente al COVID-19 en los lugares de trabajo. Objetivo: Determinar los factores de riesgo asociados a la exposición al SARS-CoV-2 durante la pandemia en Perú durante la primera y segunda ola. Material y Métodos: Estudio transversal analítico, de análisis secundario de datos. Se preguntó a diversos profesionales de la SO acerca de su exposición a trabajadores con COVID-19 durante su labor, esto a través de una encuesta virtual; a ello se asoció diversos factores sociales, educativos y laborales. Resultados: De los 2 863 encuestados, en el análisis multivariado hubo más frecuencia de exposición a pacientes COVID-19 en los sectores de minería (p=0,046), alimentación (p=0,009), en el sector público (p<0,001), manufactura (p<0,001), si el profesional realiza labor asistencial (p<0,001), si realiza evaluaciones médicas (p=0,001), que sean técnicos de enfermería (p=0,016) y que sean tecnólogos médicos (p<0,001); en cambio, hubo menos exposición a pacientes COVID-19 conforme se tenía más edad (p=0,006) y entre los que tenían especialidad en salud ocupacional (p=0,025). Discusión: Se encontraron importantes factores que se asocian a la exposición laboral de los profesionales de la SO, los que pueden servir para priorizar a los profesionales que deban cuidarse y acceder oportunamente al esquema de vacunación. Introduction: Occupational health professionals (OHP) have played an important role in surveillance, prevention and control against COVID-19 at the workplace. Objective: To determine the risk factors associated with exposure to SARS-CoV-2 during the first and second waves of the Pandemic in Peru. Material and Methods: A cross-sectional study and secondary data analysis were made. OHP were virtually asked about exposed to COVID-19 at work and its social, educational and labor factors. Results: Of the 2,863 answers, we found in the multivariate analysis: there was more frequency of exposure to COVID-19 in the mining (p value=0.046); nutrition sector (p value=0.009); in the public sector (p value <0.001), manufacturing (p value<0.001); if the professional performs care work (p value<0.001); if they perform occupational medical evaluations (p value=0.001), who are nursing technicians (p value=0.016) and who are medical technologists (p value<0.001); On the other hand, there were less exposure when they were older (p value=0.006) and among those who had a specialty in occupational health (p value=0.025). Discussion: Important factors were associated with occupational exposure of OHP professionals, which can serve to prioritize professionals who should take care of themselves and to include into complete vaccination schedule.
